ECG Challenge: Short of Breath With a Fast Pulse

A 74-year-old man with severe chronic obstructive pulmonary disease (COPD) and obstructive sleep apnea (OSA) presents to the emergency room with severe shortness of breath. He was diagnosed with a COPD exacerbation and treated with antibiotics and steroids. His pulse was noted to be irregular and an ECG was obtained.
