ECG Challenge: What Is Slowing Her Down?

The patient is a 78-year-old woman with a history of coronary artery disease and previous coronary artery bypass graft surgery. She has had chronic stable angina of effort and is being treated with an angiotensin-converting enzyme inhibitor, a beta-blocker, and a long-acting nitrate. She is seen by her cardiologist for a routine physical examination and complains of fatigue and intermittent lightheadedness. Her pulse rate is noted to be slow and an ECG is obtained.
